Information clause – e-mail correspondence

As per art. 13  of the General Data Protection Regulation of April 27, 2016 (Official Journal Of EU L 119 of 04.05.2016) – hereinafter referred to as "GDPR", the Foundation informs that:

2) Your personal data will be processed to:
a) enable e-mail contact with the controller, and to contact message addressees - art. 6 sec. 1 letter f).
b) document arrangements made with donors, prospective beneficiaries, actual beneficiaries and other persons – art. 6 sec. 1 letters b) & f).
c) receive letters and applications electronically – art. 6 sec. 1 letters b) & f).
d) provide protection against claims and pursuit of possible claims  – art. 6 sec. 1 letter f).
3) The recipients of your personal data will be: 
a) entities authorized to receive personal data under legal regulations,  
b) entities who the Foundation entrusted with personal data processing,  or is authorized to provide access thereto under effective regulations of law. 
4) Your personal data will be stored for 1 year unless 
a) a message is of material importance for pursuing claims or to provide protection against claims, in which case selected messages will be stored until the end of the limitation period,
b) a message contains significant content for the fulfillment of the Foundation's obligations under the law, in which case selected messages will be stored for a period of 6 years, unless applicable law provides for a longer period,
5) You have the right to access your data and the right to rectify, delete, restrict the processing thereof, the right to transfer data, the right to object, the right to withdraw consent at any time without affecting the lawfulness of processing conducted based on a consent given before its withdrawal,
6) you have the right to lodge a complaint with the Personal Data Protection Office, ul. Stawki 2, Warszawa,
7) Provision of personal data is voluntary but necessary to obtain a reply to correspondence sent.

Information clause for a prospective charge of the Foundation

As per art. 13  of the General Data Protection Regulation of April 27, 2016 (Official Journal of EU L 119 of 04.05.2016) – hereinafter referred to as "GDPR", the Foundation informs that:
1) The controller of your personal data is the Jerzy Bernhard Aid Foundation seated in Dąbrowa Górnicza, ul. 2) Your personal data will be processed:
a) for the purpose of art. 6 sec. 1 letter a) and art. 9 sec. 2 letter a) of GDPR i.e. to conduct a recruitment process for a charge of the Foundation, based on a relevant consent,
b) for the purpose of art. 6 sec. 1 letter b) of GDPR i.e. to take steps related to concluding an agreement as per the Foundation's Statute,    
3) The recipients of your personal data will be:
a) entities authorized to receive personal data under legal regulations,
b) entities who the Controller entrusted with personal data processing, or is authorized to provide access thereto under effective regulations of law.
c) mail operators and courier companies,
4) Your personal data will be stored until the end of the recruitment process, however not longer than 1 year, 
5) to the extent provided for by law, you have a right to request access to your personal data, request its rectification, deletion or restriction of processing, as well as a right to object to processing, and a right to transfer data.

As per art. 13  of the General Data Protection Regulation of April 27, 2016 (Official Journal of EU L 119 of 04.05.2016) – hereinafter referred to as "GDPR", the Foundation informs that:

d) for the purpose of art. 6 sec. 1 letter c) of GDPR i.e. fulfillment of Controller's legal obligations, including art. 74 sec. 2 of the Accounting Act of  September 29, 1994, and art. 26 sec. 7 of the Act of July 26, 1991 on income tax from natural persons
e) art. 6 sec. 1 letter f) i.e.  fulfillment of the Foundation's statutory tasks and protection against possible claims,
3) The recipients of your personal data will be:
a) entities authorized to receive personal data under legal regulations,
b) entities who the Controller entrusted with personal data processing, or is authorized  to provide access to the said data under effective regulations of law.
c) banks – in connection with payment handling,
d) mail operators and courier companies,
4) Your personal data will be stored for the period of 6 years from agreement termination/expiry, and in the case of data processed based on a relevant consent, until it is withdrawn;  personal data processed for settlement-related purposes  will be kept by the Controller for the period of storing accounting and tax documents imposed by regulations of law; data processed for the purpose of pursuing claims (e.g. in court proceedings), will be stored for the period of limitation of claims resulting from the provisions of the Civil Code;
5) to the extent provided for by law, you have a right to request access to your personal data, request its rectification, deletion or restriction of processing, as well as a right to object to processing, and a right to transfer data.
6) you have a right to lodge a complaint with the Personal Data Protection Office ul. Stawki 2, Warszawa if you believe that data is processed in violation of GDPR,

As per art. 13  of the General Data Protection Regulation of April 27, 2016 (Official Journal of EU L 119 of 04.05.2016) – hereinafter referred to as "GDPR", the Foundation informs that:

a) for the purpose of fulfilling the Foundation's statutory tasks and to send you information on the Foundation's activities and express gratitude  – under art. 6 sec. 1 letter f)  of GDPR.

d) for the purpose of service performed, agreement concluded, or in order to take action before it is fulfilled or concluded (art. 6 sec. 1 letter b) of GDPR)
e) to fulfill the legal obligation imposed on the Controller (art. 6 sec. 1 letter c) of GDPR)
f) to pursue claims  – legitimate interest of the Controller (art. 6 sec. 1 letter f) of GDPR)

As per art. 13  of the General Data Protection Regulation of April 27, 2016 (Official Journal of EU L 119 of 04.05.2016) – hereinafter referred to as "GDPR", the Foundation informs that:

2) the provided personal data will be processed for communication purposes based on a separate consent given in a PIT form filed with a competent Tax Office under art. 6 sec. 1 letter a) of GDPR;
3) Your personal data will be processed to fulfill the legal obligation imposed on the Controller (art. 6 sec. 1 letter c) of GDPR)
4) The recipients of your personal data will be:
a) entities authorized to receive personal data under legal regulations,

b) related to electronic payments made by payment terminals – in order to accept electronic payments for administrator services due to the processing requirement, will be processed for a period necessary for current settlements, 
c) included in accounting and tax documentation and in banking systems and documents – will be processed for the period of 5 years from the end of a given calendar year, 
d) processed in order to pursue possible claims or defend against claims - will be processed for a period of 6 years from the date of completion of the provision of a specific delivery or service, acceptance / provision of support, in the case of pending proceedings, this period may be extended until the proceedings are finalized and until the expiry of new limitation periods,
e) processed on the basis of a consent granted will be processed until it is withdrawn,
5) Pursuant to GDPR you have the right to:
a) access your data and receive a copy thereof,
b) request rectification (correction) of personal data - if it is incorrect or incomplete,
c) remove data, if there are no grounds for its further processing,
d) request  a reduction of personal data processing,
